Application Form Instructions
- Select program of interest hyperlink below
- Fill out application contact information on the form
- Select elective courses listed to meet program requirements
- Select “Submit” button, this sends CPS’s an electronic copy of your application
- Select “Print” button, this prints a hardcopy of your selections
- Go to “Programs” page of the website and select your program and print a hardcopy
- Take the “Application Form” and “Program Value Proposition” to your caseworker
